Position Summary: The Postpartum Maternal/Child Health Care Coordinator is accountable directly to the Group and Education Coordinator. This role provides group education focused on postpartum health and wellbeing, infant care, and parenting to patients participating in group care. The candidate captures patient goals as related to the group education provided and provides a closed-loop connection to medical or social supports needed to reach goals. The candidate maintains the structure of education groups by planning and executing the daily tasks required to coordinate sessions, including active recruitment of patients and activities to support the retention of participants in the group. This is a full-time position working onsite 4 days/week and remotely 1 day/week from our Family Health and Birth Center (2120 Bladensburg Rd NE) and our Conway Health and Resource Center (4 Atlantic St SW).
 

 The pay rate for this position ranges between $25.00 - $25.96 per hour and the offer amount is determined by the candidate's education, qualifications, and experience.  Indeed provides their own estimated salary calculator and is not affiliated with COH's range.

For over 40 years, we have provided healthcare, housing, and supportive services for under-resourced, underserved and people experiencing homelessness in Washington, DC. As a Federally Qualified Health Center, we provide medical, dental, emotional wellness, and care coordination services for the whole family at three locations in DC. Community of Hope also strongly emphasizes maternal and child health, with midwifery practice and the only free-standing birth center in DC. In 2020, Community of Hope provided about 28,400 medical visits, 7,000 dental visits, and 10,000 behavioral health visits for about 11,000 patients. Community of Hope provides community walk-in COVID testing and COVID vaccines. Community of Hope is also one of the largest providers in DC of housing and support services for families and individuals experiencing homelessness. Through providing these programs, we live out our mission to improve health and end family and individual homelessness to make Washington, DC, more equitable.

Job Requirements
Highlighted Duties and Responsibilities:
Provides education and assists patients in the development of short- and long-term goals with a particular focus on postpartum self-management of care, infant care and wellbeing, and parenting to patients participating in postpartum group care.
Acts as liaison to patients and caregivers, providers, clinical staff and program leadership, developing and analyzing innovative approaches to postpartum support for patients. Provides relevant information to support the initiatives as requested.
Facilitates at least six infant massage parent education groups annually using the Infant Massage USA mode.
Supports registration for breastfeeding/chest feeding groups and distribution of incentives following each group.
Co-facilitates multiple postpartum group care appointments with a midwife or FNP on a weekly to monthly basis using the identified interventions (i.e. Centering Parenting, play groups, etc).
Helps to develop postpartum group calendars ensuring that they are accurate and representative of all programming documenting, communicating any changes to supervisor, COH staff, group guests, and group participants.
Assesses barriers to adherence to group schedule, maintaining contact with postpartum patients regarding missed group appointments to ensure that follow up is completed.
Actively recruits postpartum group participants by connecting with patients before the conclusion of each CenteringPregnancy group to offer postpartum group enrollment.
Documents all patient encounters, including face to face, telephone, and electronic communications within two business days. Documents/populates all group materials provided to group participants in the electronic record.
Co-facilitates, attends, and assists CenteringPregnancy groups when necessary to provide back up to the prenatal group care coordinator.
Represents Community of Hope at health-related community functions as requested.
Performs other duties as requested by supervisor.
Requirements

Minimum Qualifications:
High School Diploma/GED required.
Bachelor’s degree in public/community health education, maternal & child health, or social work preferred.
2 years of practical experience related to maternal and child health, doula and/or childbirth education, home visiting, or related field.
Experience providing education and coordinating health programs and initiatives in a group or individual setting with a specific focus on maternal & child health preferred.
Ability to express enthusiasm about breastfeeding and a desire to help lactating individuals enjoy a positive experience.
Knowledge of the CenteringPregnancy and/or Parenting model (preferred) or experience working with perinatal groups.
Commitment to work with underserved populations and ability to work in non-profit environments.
Demonstrates cultural competence in communicating with low-income populations required.
Ability to travel between COH sites or relocate to a different COH site on a full or part-time basis required.
Ability to work Saturdays and evenings required.
Strong interpersonal skills are required.
Ability to be creative, innovative, and multi-task.
